Services

   Home Health and Hospice Care of Somerset Hospital provides in
   home care under the direction of the patient's physician.

   Upon admission to Home Health, the patient is assessed and a plan
   of treatment is developed with the patient's assistance.  The plan
   of treatment is submitted to the physician for approval.  

   Home Health Hours of Operation:

   4Office hours are 8:00 AM to 4:30 PM , Monday through Friday.

   4Service is available 7 days a week, 365 days a year.

   4On call nurses are available for admissions, scheduled visits,
      and patient phone calls.

   In Touch Hospice Hours of Operation:

   4Office hours are 8:00 AM to 4:30 PM , Monday through Friday.

   4Hospice nurses are available 24 hours per day, 7 days a week,
     365 days a year.

   Services include:

   1.     Visiting Nurses:
           n Health care assessment and symptoms control
           n Diabetic and other diseases teaching
           n Medication Teaching
           n Diet Teaching
           n Post surgery care
           n Wound care
           n  Intravenous therapy
           n In home chemotherapy

   2.     Other Services include:
   n Physical Therapy
   n Occupational Therapy
   n Speech Therapy
   n Medical Social Work
   n Home Health Aide Services
   n Psychiatric Nursing
   n Infant/Maternal Early Discharge Program
   n Hospice Care through "In Touch Hospice"
