Referrals
Referrals may be made by the
physician, hospital or nursing home
discharge planner, family
members, caregivers, and the patients
themselves.

HOW TO REFER YOURSELF OR A
FAMILY MEMBER
If the patient is in the hospital
·
Tell the doctor of discharge planner/social worker
that you want a home health or hospice referral.

Tell them that you want Somerset Hospital Home Care
or Hospice Care, if that is your choice.
Remember that everyone has the right to choose the agency
that they want.

Before the patient is discharges from the hospital
or nursing home, check with the nurse to be certain a home care
order has been written.
·
If you choose Home Health & Hospice Care, the
patient will be contacted within 24 hours after discharge.
If the patient is at home (no
hospital stay)
1.
Call the patient's doctor and, if possible, make an
appointment for the patient to see the doctor.
2.
Ask the doctor to call Home Health & Hospice
Care to make a referral.
3.
If the doctor is not sure if the patient qualifies
for home health or hospice an evaluation visit can be made to
determine eligibility.
4.
If unable to have the doctor see the patient, call
Home Health & Hospice Care and we will call the doctor for the
referral.
