   Eligible Patients for:
   Home Health:
   Anyone living in Somerset County who meets the following criteria:

   1.Requires a qualifying intermittent service which is one of the   
    following:
         4 Nursing,
         4 Physical Therapy, or
         4 Speech Therapy

   2.Patient is essentially homebound due to a medical or
    ambulation condition.

   3.The patient is under the care of a physician who is willing to order
    the services and care needed.

   Hospice

   1.Patients who have a terminal diagnosis (Cancer or Non-cancer).

   2.Patients must have a primary care physician.

   3.Patients should live with family or friends who can provide basic care.

     4 Diagnosis related to reason for the referral
     4 Any specific orders i.e. blood work, wound care, PT, OT
     4 Medical Insurance Type and Number
