"In
Touch" Hospice
The concept of
Hospice shows a concern for the total family, and this
is what makes
Hospice care different from traditional forms of health
care.
Ideally, the patient will be cared for at home by the family
as
long as possible. The
Hospice team, with the support of the patient's
physician, can help
the dying patient and the family confront their
problems, adjust to
them, and concentrate on achieving quality in the
dying person's
remaining lifetime.
"In
Touch" Hospice Care is a non-profit community service provided
by
Somerset
Hospital
free of charge under the direction of the patient's
physician.
The purpose of the service is to bring comfort into the home
at this most distressing time.
WHAT SERVICES
DOES HOSPICE PROVIDE?
Health care and emotional support for the patient and family.
This includes:
4Regular visits by nurses trained in the Hospice
concept.
4Consultations and cooperation with the patient's
physician to
provide pain and symptom control.
4Visitation by a social worker to assist with meeting
the patient's
social and emotional needs.
4Availability of inpatient care should the patient's
condition
warrant such.
4Volunteers who have become meaningful friends willing
to listen,
share, and help ease the problems of a terminal illness.

Bereavement
Services
The Hospice
concept maintains the belief that its care does not end
with the
death of a patient. Therefore,
contact with the family during
the bereavement period is continued.
WHO IS SERVED?

considered terminally ill and live in

County.
Patients should like
with family or friends who can provide basic care.
Patients must have a primary care physician
