Advance
Directives
Home Health and
Hospice Care believes that all patients who are
mentally capable of
making decisions about their health care and
treatment have the
right to do so. This
includes the right to refuse
care and treatment.
Home Health and
Hospice Care recognizes written Advance Directives
in the form of a
Living Will, Durable Medical Power of Attorney, or
similar documents
conveying the patient's wishes.
At the time of
admission, the nurse will ask the patient for written
Advance
Directives. If the
patient has one, the nurse will request a
copy for the patient's
medical record. If the
patient does not have a
written Advance Directive, the nurse will
give the patient a copy of
"Making Decisions About Your Care
and Treatment, Your Rights as a
Patient in Pennsylvania" and a
blank sample of a Living Will.
